Welcome to the Online Provider locator page.  To get the most from your health plan benefits, Valley Baptist Health Plans strongly urges the use of participating providers.

This page has been set up to help you locate participating physicians, hospitals, clinics, and other participating providers within your plan network.

You can download and print a complete network provider directory for your plan using the Network Code located on the top right side of your Member ID Card.

Texas Tru Choice Network
If you have a covered dependent attending college or a court ordered dependent living outside of our service area, but still living within the State of Texas, search for a provider for that dependent using our Texas True Choice Network. If your dependent attending college, or your court ordered dependent lives outside of the State of Texas, please call our Customer Service department for assistance in locating a provider for that dependent. You can locate a physician in a variety of methods using a name, zip code, or specialty.
